Health Form
Use the Health Form for Monthly Campouts and Summer Camp for Adults and Scouts
The Boy Scouts of America requests as of January 1, 2010 that all youth and adult members have annual medical evaluations by a certified and licensed health-care provider. Parts A and B are to be completed annually by ALL BSA unit members. Part C is required with Parts A and B for any event that exceeds 72 consecutive hours. Application Forms
Application Forms are those provided by the Boy Scouts of America (BSA) Northern Star Council. Note: There are many more Application Forms available for download at the Northern Star Council website.
